If you decide to come to Germany for treatment all you have to do fill the form below stating all your requirements and needs. Send the form by fax or by E-mail and leave the rest to German Medical service.

we will contact you for details of bookings and approximate cost of your treatment " If you then wish to proceed, you have to confirm immediately " Once you confirm your bookings, an amount of one week in advanced should paid, and the rest of the rent should be settled two weeks before arrival to Germany .
